Fernandes equaled the single-season competition record of 20 assists, a feat that placed him ahead of an eight-man shortlist.
The list included Arsenal midfielder Declan Rice, Arsenal defender Gabriel, Arsenal goalkeeper David Raya, Manchester City striker Erling Haaland, Manchester City winger Antoine Semenyo, Brentford forward Igor Thiago, and Nottingham Forest midfielder Morgan Gibbs-White.
The 31-year-old previously turned down a lucrative contract offer from the Saudi Pro League to remain at Old Trafford.
He recorded eight league goals and 28 total goal contributions across 37 matches, helping Manchester United secure Champions League qualification.
This award follows his selection as the Football Writers' Association's men's footballer of the year.
Fernandes is the first Manchester United player to claim that specific honor since Wayne Rooney in 2010.
United's top-four resurgence occurred under Michael Carrick, who took over after the dismissal of Ruben Amorim in January.

Carrick will remain as permanent manager beyond the season.
"I hope he's one that can take us back to the top of the Premier League because this is what all the players want," Fernandes said.
Arsenal midfielder Declan Rice was the primary runner-up after playing 35 of 36 league matches. He helped the Gunners clinch their first Premier League title in 22 years.
Manchester City forward Erling Haaland finished as the league's top scorer with 26 goals and nine assists in 35 appearances.
His teammate Antoine Semenyo contributed to City's title challenge after a January transfer from Bournemouth.
Arsenal goalkeeper David Raya secured the Golden Glove award with 18 clean sheets.
