Julio Enciso opened the scoring for Paraguay with a header in the 42nd minute, beating German goalkeeper Manuel Neuer.
Kai Havertz equalized for Germany early in the second half with a header, his third goal of the tournament.
In extra time, a header by Germany's Jonathan Tah in the 102nd minute was disallowed after a VAR review determined Paraguay goalkeeper Orlando Gill was interfered with.
The match went to a penalty shootout, where Germany's Havertz, Nick Woltemade, and Tah all failed to convert their attempts.

Paraguay's Mauricio, Gustavo Gomez, Matias Galarza, and Jose Canale successfully converted their penalties to secure the 4-3 shootout victory.
Brazil Also Advances
In another Round of 32 match in Houston, Brazil came from behind to defeat Japan 2-1.
Kaishu Sano gave Japan the lead in the 29th minute, but Brazil equalized through a header from Casemiro and won it with a late goal by Gabriel Martinelli.
