The couple has suffered drastic weight loss and debilitating weakness.
The British Embassy in Tehran sent medication, glasses, books, and hygiene products, but Iranian authorities withheld these items, the family said.
In a letter published Tuesday, the family expressed their emotional distress and pleaded for the couple's survival.
"Please remember that you are loved by so many people," the letter read.
"We need your voices, your presence, your laughter, and your company."
The letter concluded: "We need you well. We need you alive."

The family criticized the withholding of supplies as a breach of formal agreements and urged British officials to intervene for medical access and repatriation.
"Act without delay," the family urged.
The couple remains cut off from communication with relatives and their children.
Lindsay Foreman's son, Joe Bennett, planned to meet with lawmakers at Parliament on Wednesday.
"They are starving themselves because every other route has been closed to them," Bennett said.
Iranian authorities detained the couple in January 2025.
